Unconventional dialysis access poses life threatening risk for dialysis patients

Dr. Achintya Sharma, Consultant & In-charge, Vascular & Endovascular Surgery

Panipat: With the significant rise in the incidences of renal failure, those requiring hemodialysis have increased in recent times. Pertaining to the fact that, on occasions, the dialysis access may stop working well or cease working altogether attributing to a narrowing or blockage (clot) of one or more of the blood vessels in the fistula or graft. When these dialysis access complications occur, it is essential that the vascular access is working correctly and can maintain adequate blood flow to and from the haemodialysis machine during the treatment for normal dialysing.

In order to raise awareness among dialysis patients about the precautions to be taken care of during and post dialysis session, to avoid vascular access complications during dialysis, doctors from Max Hospital Shalimar Bagh organised a public session today. The session was orated by Dr. Achintya Sharma, Consultant & In-charge, Vascular & Endovascular Surgery.

Max Hospital, Shalimar Bagh also runs its weekly OPD services for Nephrology and kidney transplant medicine at Haiderabadi Hospital, where Dr Manoj Arora Principal Consultant & Unit Head, visits for consultations. While the vascular surgeons and nephrologists work in tandem as a team, these exclusive OPD services will be a boon for the patients in the region, to avail access to expert opinion and consultations. With improved connectivity and reduced geographical barriers, complicated cases of dialysis patients can be referred to Max Hospital Shalimar Bagh.

The most common complications associated with patients undergoing dialysis include AV dialysis access failure, blocked or fluctuating flow rate (inflow and outflow of the blood), ruptured fistula causing blood loss, cephalic vein stenosis, formation of pseudoaneurysm, or abscess can be life-threatening with unstable vital signs, and any delay in operation can significantly enhance the chances of morbidity and mortality.

With the recent advancements made in the field of endovascular intervention techniques, patients with a multitude of ailments undergoing dialysis can be treated, so that the patients have a better quality of life.

“Sometimes, however, the blood flow rate through your access is inadequate due to a narrowing of a blood vessel in the fistula or graft, or it may suddenly stop working altogether due to cephalic artery stenosis, ruptured fistula, or blocked inflow in the arteries. If a patient has a ruptured fistula, we ligate it. AV Graft Ligation is frequently an emergency procedure requiring immediate closure. While Cephalic vein stenosis is also a common cause of AV dialysis access failure, are most of the time difficult to treat with venoplasty. In such cases a procedure known as angioplasty and ballooning is required to reopen the access and continue receiving dialysis treatment.” Said Dr Achintya Sharma.

The team at Max Hospital Shalimar Bagh is enriched with surgeons with significant experience with all aspects of treating a patient pertaining to all types of vascular conditions including diagnostics, medical treatment, reconstructive vascular, surgical and endovascular techniques. With their advanced knowledge and skills, patients are provided with comprehensive care as per their needs. The vascular surgery team is well experienced in performing all kinds of fistula work as in unconventional access, Thigh – loop graft, basilic & Brachial vein transposition, and is a one-stop solution with centre of excellence where all CKD patients are well taken care of, who multiple AV access failure requiring lifesaving dialysis access.

Many patients with chronic renal failure undergoing hemodialysis are required to create an Arteriovenous Fistula (AV Fistula) which acts as a lifeline in filtering their blood. Thus, it is imperative that the fistula or graft is working properly, and in any case of ineffective functioning, the doctors will perform a fistulogram, which is an imaging technique to visualize the severity and location of any narrowing/blockage in the fistula.

Timely intervention helps in better management of vascular condition and the advancements made in vascular procedures are getting better with time. this is a substantial step taken by Max Healthcare towards ensuring that such patients in the region have easy and speedy access to quality healthcare services.
